FAMILY BASED SUPPORT SETTING

This service is provided in the home of the person receiving services and or in the local community. The activities provided through this support setting are designed to increase skills and behaviors necessary to achieve greater independence and personal choice, thereby allowing for greater inclusion into their community.

The services employed in these settings will be Person Centered Support-Family, Person-Centered Supports Agency, and Respite-Agency by definition of the IDDW Manual.  Providers in this setting include family members who are employed as Family Support Providers through Hopewell as well as ISP staff through Hopewell. All services will be provided in accordance with the IDDW guidelines and limitations.
